This can be:

  • A person experiencing harm or any type of abuse.

  • Loss or damage of property.

  • Something that could have caused harm.

  • An accident.

  • Illegal activities.

  • An incident you didn’t witness but are made aware of.

This includes:

  • Reporting incident to relevant personnel

  • Protecting evidence.

  • Notifying an impacted person’s support person or family.

  • Contacting police or other relevant authorities.

Timeframe:

  • Complete the incident report form within 24 hours of incident occuring. Form located above.

Actions

  • Ensure the safety and wellbeing of persons involved.

  • Render first aid if requited.

  • Call 000 if required.

  • Report incident to Sparrow Collective Management as soon as possible and no later than 2 hours following the incident.

Actions

  • Identify if incident is a REPORTABLE incident:

    • Death of a person with disability.

    • Serious injury to a person with disability.

    • Abuse or neglect of a person with disability.

    • Unlawful physical contact with, or assault of, a person with disability.

    • Sexual misconduct committed against, or in the presence of, a person with disability; including grooming of the person for sexual activity.

    • The unauthorised use of a restrictive practice in relation to a person with disability.

  • Let Sparrow management know when you have completed the form.

Management Timeframe for Reportable Incidents

  • For reportable incidents, management will notify the NDIS Commission no later than 24 hours after becoming aware of the incident.

Questions

Management will consider the following questions regarding the incident:

  • Could it have been prevented?

  • How well was the incident managed?

  • What, if any, remedial action needs to be undertaken to prevent further similar incidents, or minimise their impact?

  • Do other persons of bodies need to be notified?

Requirements

Under the NDIS incident management requirements we will record the required information and details surrounding the incident.

We will make this information available tot he NDIS Commission, auditors and qualified investigators.

Actions

The consideration of these points will determine the quality improvement actions implemented in the company.

  • Providing ongoing support to those impacted and/or ensuring their ongoing wellbeing and safety.

  • Disciplinary action or dismissal.

  • Identifying and implementing practice and procedure imporvements.

  • Reviewing risk assessment measures.

  • Identifying and taking corrective action to prevent recurrence of incidents.

Sparrow Collective will record the outcome of their assessment on the Incident Report form and Incident Register, and amend any necessary policy and procedure documents.
